Appointment Arrival:
We kindly request that patients arrive 10 minutes prior to their scheduled appointment to complete any necessary paperwork ensuring the full appointment can be dedicated to care.
Late arrivals may result in a shortened appointment time or rescheduling if more than 15 minutes late.
Cancellation Policy:
We understand that unforeseen circumstances may arise. To avoid fees, please provide at least 24 hours' notice for appointment cancellations across all services offered.
Late cancellations or no-shows may be subject to a cancellation fee of $50.
Recurrent missed appointments may require prepayment for future appointments.
Cell Phone Use:
To maintain a quiet and respectful environment, we kindly request that cell phones be turned off or set to silent mode while in the waiting area and treatment rooms.
Children Policy:
For safety reasons, children must be supervised during appointments, particularly when procedures involve sterile or biohazardous materials. We encourage arranging additional childcare when possible. If children must accompany you, please ensure they do not interfere with medical procedures or equipment. Some treatments may involve needles, scalpels, or other instruments that could be distressing for children to observe."We appreciate your understanding and cooperation in making our medical practice a welcoming and safe space for everyone. Gift Certificates:
Gift certificates for medical services are available for purchase and can be used toward eligible treatments.
Gift certificates are non-refundable and must be presented at the time of service.
Insurance and Billing:
Wellness & Hormones Avenue offers cash based laboratory tests drawn by our provider or external laboratory office where prices have been negotiated. Patients typically pay cash to the Practice but some patients prefer to use their health insurance for such tests. The Practice will allow you to use your health insurance if you so choose. Your health insurance provider however may not fully cover the costs of these laboratory tests, or you may have a high deductible that you need to meet before insurance benefits apply.
Patients are responsible for verifying insurance coverage and understanding any out-of-pocket expenses prior to lab draws. The Practice does not file claims on behalf of patients.
Medical Records Requests:
Patients may request copies of their medical records. A processing fee may apply. Requests are usually processed within 7-10 business days.
Prescription Refills:
Prescription refills are addressed during scheduled appointments. For refills, please contact your pharmacy 1-2 weeks before running out. They will notify us if your prescription requires renewal. Direct requests to our office may take up to 72 hours to process.
Laboratory Work:
Labs can take 3-10 days for results to be sent to us after your lab draw. The provider then has to interpret the results. Please allow up to 10 days to hear back about your lab test results for any immediate health concerns. Otherwise labs will be discussed during your next appointment. Not all laboratories have the same turnaround time.

Emergency Procedures:
